VIDEO SURVEILLANCE

Inside the “Comprehensive Fertility 360” facilities there are security video cameras, which can capture images and/or video of the activities carried out in common areas for your safety and that of the people who visit us and work in said facilities.

What type of data may be collected?

To achieve the aforementioned purposes, the following personal data will be processed: full name, address, telephone number, email address, marital status, age, sex, nationality, date of birth, name, address and telephone number of a family member that you designate as a responsible family member. and with whom we can communicate in case of emergency; If applicable, your policy number and other information related to the medical expenses insurance you have contracted. SENSITIVE PERSONAL DATA

In order to provide medical-hospital care and in accordance with applicable health legislation, the sensitive personal data required for this purpose will be requested: religion, current state of health, past and present illnesses, family history, symptoms, relevant pathological history, health history, and in some cases, when required for your adequate medical care, sensitive personal data such as your sexual preference and genetic information may also be processed (the latter data, for clinical diagnostic studies that you or your treating physician have requested).

DATA TRUTHFULNESS

Users guarantee that the personal data provided are true and are responsible for communicating to “Fertilidad Integral 360” any modification thereof. ARC PROCEDURE AND REVOCATION OF CONSENT

For the exercise of your ARCO Rights and/or the revocation of your consent for the processing of your personal data by “Fertilidad Integral 360”, we would appreciate submitting a request (“Request for Exercise of ARCO Rights”) to the Privacy Committee, to the email email privacy@fertility.com, accompanying the following information:

If the owner is not the one who presents the request, the document that proves the existence of the representation, that is, a public instrument or power of attorney signed in front of two witnesses, along with identification of the owner, the representative and the witnesses (credential for vote, valid passport, professional ID or immigration document). The Privacy Committee has at your disposal the formats that are recommended for the exercise of ARCO Rights, so the one corresponding to your request will be provided to you at the time you request it from the Committee, by any means.

“Fertility Integral 360” may deny the exercise of ARCO Rights, in the following cases:

• When the rights of a third party are violated;
• When there is a legal impediment or the resolution of a competent authority, which restricts your ARCO Rights;

The exercise of “ARCO Rights” will be free, but in the event that two or more applications are submitted in a period of less than twelve months, “Fertilidad Integral 360” may charge for the issuance of copies of documents.

The applicant must cover the justified shipping costs or the cost of reproduction in copies or other formats.
